Background/Question: Considering the increasing need for crisis intervention services in Austria, we ask what specific measures social work can use to provide professional crisis in-tervention, health promotion and prevention within the Austrian healthcare and social care landscape. The focus is on the potential of primary care centres (PCCs), as we see specific advantages in PCCs such as prompt support close to home, easy-access, continuous relation-ship management, multi-professional services, and networking in the local community. The Trouble Management Programme (TMP) was conceptualised by incorporating aspects from established programmes such as Disease Management Programmes, Case Management or Social Prescribing, as well as theories, methods and specialist knowledge of social work and crisis support. Methods: The needs assessment and literature research, including an in-depth, topic-oriented literature analysis, was followed by empirical data collection in the form of an expert workshop with a trialogue approach (crisis support and PCC social work professionals, crisis experienced persons, and their relatives). The concept for TMP was generated by analysing and processing data from specialist literature, specialist lectures and the empirical survey. Results: The Trouble Management Programme (TMP) is an instruction manual and social work-related support programme that can be used in primary care centres. TMP contains guidelines for professionals and participation documents for clients/patients, both in acute cri-ses and for longer-term stabilisation and further care or health promotion, as well as concrete suggestions for quality assurance measures. The programme claims to focus on the individual problems and resources of the users despite structured procedures. Conclusions: The multi-professional team and other specific advantages of primary care cen-tres have enormous potential to sustainably expand the care landscape in terms of crisis sup-port and psychosocial health promotion.
